(1) With reference to the Service of Ms Emily Roper, Former Director General of the Department of the Premier and Cabinet (DPC), please provide the following:
(a) Total Payout in Dollars;
(b) Payout in dollars for Termination Portion Only:
(i) Provide reference to Statute or Regulation for calculation of 1(b) for method of calculation;
(c) Payout in dollars lieu of Long Service Leave portion:
(i) Provide reference to Statute or Regulation for calculation of 1(c) for method of calculation;
(d) Payout in dollars for accumulated Annual Leave portion;
(e) Payout in dollars for accumulated paid personal (incl Sick) Leave portion;
(f) Payout for any item, not including Leave or Termination entitlements ( eg Specify as per Vehicle, travel, ex gratia etc);
(g) Effective Date of Service Completion, ( last day at work in the role);
(h) Start date for calculation of Service entitlements;
(i) Finish Date for calculation of Service entitlements;
(j) Accumulated annual leave taken between 1 (h) and 1 (i);
(k) Accumulated Long Service leave taken between 1(h) and 1 (i);
(l) Accumulated paid personal leave taken between 1 (h) and 1(i); and
(m) Confirm if any maternity or other special leave was taken between 1 (h) and 1(i)?
(2) Annual Pay, leave, travel and allowance entitlement for the position at the time of termination, as per the following:
(a) Annual Salary;
(b) Annual Vehicle Allowance;
(c) Rate of SGC or Employer Super contribution (%); and
(d) Any other entitlements as part of the Emily Roper Salary Package at the time of termination or resignation?
(3) Provide information of any non financial benefits provided to Ms Emily Roper eg mobile telephone, broadband at home and working from home entitlements?
(4) Provide date of tthe termination notice sent to Ms Ropedate of effective termination and period of notice provided?
(5) Provide period of notice provided?
