(1) Since the tabling of the Coroner’s report into the death of Jamie Frederick Ginn, what formal directions, instructions, or revised procedures have been issued by Western Australia Police to ensure compliance with:(a) the “Death or Serious Incident at the Perth Watch House (PWH) – Initial Response” checklist, including the preservation and seizure of potential forensic evidence; and(b) basic and strip search policies at the Perth Watch House?(2) On what dates were the directions referred to in Question 1 issued, and what training or guidance has been provided to Watch House staff, Police Auxiliary Officers, custodial health staff, and attending paramedics to support their implementation?(3) What audit, assurance, or compliance activity has been undertaken by Western Australia Police since the tabling of the Coroner’s report into the death of Jamie Frederick Ginn to verify adherence to the requirements referred to in Question 1, and what were the findings?

(1) (a) In February 2026, the WA Police Force established a Senior Sergeant Operations Manager role at the Perth Watch House (PWH) to ensure compliance with all operational requirements and staff adherence to policy and procedures. (b) Refer to (1) (a). (2) On 4 September 2025, 3 months prior to the publication of the Coroner's report, all supervisors at the PWH were given formal directions reaffirming the existing custodial procedures and compliance with the "Death or Serious Incident at the PWH - Initial Response" checklist. This process was further re-enforced after the Coroner's findings on 17 December 2025. (3) The WA Police Force has undertaken targeted assurance and complaince at the PWH to verify adherence to the requirements referred to in Question 1. In February 2026, the WA Police Force established a Senior Sergeant Operations Manager role to ensure compliance with all operational requirements and staff adherence to policy and procedure.
