For the period 1 January 2025 to 31 December 2025, in relation to voluntary assisted dying in Western Australia, I ask:(a) how many persons died as a result of voluntary assisted dying services;(b) how many First Requests to access voluntary assisted dying were refused;(c) of the persons who died receiving voluntary assisted dying services, how many were:(i) age 18 - 24;(ii) age 25 - 34;(iii) age 35 - 44;(iv) age 45 - 54;(v) age 55 - 64;(vi) age 65 - 74; and(vii) age 75 or older; and(d) of the persons who died receiving voluntary assisted dying services, what is the breakdown recorded in each underlying disease, illness or medical condition category?
