answer on 17 March 2026 to my question without notice 161 wherein the
Premier advised the house that the department had conducted an internal review
following the distressing incident and that a range of changes had been
implemented.
(1) Who within the department conducted
the review?
(2) When did it commence and when was it
completed?
(3) How many changes were recommended?
(4) Who approved the changes?
(5) Have they all been implemented and,
if not, why not?

Police Force.
(2) The review commenced in December 2024
and was completed in February 2025.
(3) There were five recommendations.
(4) The Department of Justice and the
Western Australia Police Force.
(5) All recommendations have been
implemented.
